All patients need to complete the two forms available for download from this site. One is a Medical History form that may be more detailed than is customary. Remember, you will be working with a naturopathic physician who has a different and more vested approach to your healthcare than a traditional medical doctor. All aspects of your health history play a role in who you are today. In addition, there is an Informed Consent form for your review and signature.
All paperwork can be completed and brought with you to your first appointment.
